In general, patients who travel by air are divided into two general categories:

  • A group whose travel is accidental due to their illness and they travel by air for pilgrimage, tourism, etc. Most of the sick passengers are in this group and usually there is no problem for them to fly.
  •  The second group is people whose illness is the main reason for their trip and they travel for treatment or to get expert advice, etc. Although this group of patients is a very small group of air passengers, they are usually unable to take care of themselves during the flight and have certain needs. Also, flight conditions can potentially cause adverse effects on their illness and condition.

Physiological changes created under the influence of factors such as pressure and oxygen reduction are intensified and can cause the condition of susceptible patients to become critical. Also, the physical limitations of the airplane cabin and the complications and mental emotions caused by the flight can create problems.

In general, all patients who are in acute and unstable conditions are required to obtain medical permission for their air travel. The air medical unit of the airline companies is responsible for making decisions regarding this category of passengers and only they can issue air travel permits for patients and specify under what conditions and with what facilities or restrictions the patient can travel by air. In the meantime, the opinion of the attending physician can only be considered as a recommendation and the final decision is the responsibility of the aviation medicine department.
